If you are interested in reading some real-life reports on incidents with a broad cross-section of topics such as fatigue, GPS errors, ground errors, navigation etc and soforth, check out this link http://asrs.arc.nasa.gov/report_sets_nf.htm it's quite a good read and interesting to note some of events that happen out there (but hope it doesn't put you off flying!
